Information for Foreign Patients

Outpatient

Outpatient Visit For Consultation

Koseikan has been certified as a "Community Medical Support Hospital" that handles specialized treatment, advanced testing, inpatient treatment, emergency medical care, etc., and has been selected as a "Priority Medical Institution for Referrals".
We require referrals from other medical institutions (primary care doctors', neighboring hospitals or clinics) for consultations.
Thus first-visit patients should bring a letter of reference.
When you come our hospital without referrals , you will be requested to bear a fee of ¥7,700 (including tax) as a "Fee for Treatment of Patients' Choice" in addition to the medical fee.

To confirm patient safety, we will ask for your full name and date of birth during the consultation.
Please tell your doctor about your medical history, allergies, and any medications you are taking at the time of consultation.

First-visit patients

Please bring the following

Japanese health insurance card

Passport

Residence card or the number of that

Referral letter (if applicable)

Emergency contact

  1. Please come to the reception desk* on the 1st floor, at counter No. 2 or 3.
    We will issue you a patient registration card and provide you with a number tag.
  2. Please hand your patient registration card to the outpatient reception of the department where you will be examined.
  3. Please wait near the outpatient reception desk where you will be examined until your reception number is displayed on the waiting board.
  4. When your number is displayed, please enter the corresponding examination room to be examined.
  5. After your consultation, please submit the reception slip to the accounting desk at No. 6.
  6. Please pay at the automated teller machine on the 1st floor when your billing number is displayed.
    Persons with walking difficulties, wheelchair users, and those pushing strollers should pay at the priority desk
  7. After paying, please fill out the questionnaire at desk No. 8.

Attention

If you have any concerns about your Japanese, please request the use of a translation tablet at the 1st floor general reception desk.

Also, you can request medical interpreters volunteer in advance.

If necessary, please call us at least 1 week prior to your visit to Koseikan.

If you forget your passport or consultation ticket, please tell the general reception desk on the 1st floor.

Follow-up patients

Follow-up patients must come to the hospital by the time of your appointment.

Reservation slip

Patient registration card

  1. Follow-up patients are accepted at the 1st floor return visit reception machines*.
  2. Please put your patient registration card into the insertion slot of the return visit reception machine.
  3. You will get a patient registration card and a reception slip.
    Please hand them to the outpatient reception of the department where you will be examined.
  4. Please wait near the outpatient reception desk where you will be examined until your reception number is displayed on the waiting board.
  5. After your consultation, please submit the reception slip to the accounting desk at No. 6.
  6. Please pay at the automated teller machine on the 1st floor when your reception number is displayed.

Attention

When it has been more than 6 months since your last visit to Koseikan, we will regard you as First-visit patients.

If you forget your patient registration card, please tell the receptionist at No.4 desk on the 1st floor.
