Handling of Personal Information

Purposes of Use of Personal Information at the Hospital

■ To provide medical care

  • Providing medical services at the Hospital.
  • Cooperating with other hospitals, clinics, midwifery centers, pharmacies, home care stations, nursing care service providers, etc.
  • Answering to inquiries from other medical institutions, etc.
  • Seeking opinions and advice from outside physicians and others for patient examination.
  • Outsourcing of specimen testing services and other services.
  • Explaining of medical condition to family members, etc.
  • Other using related to the provision of medical care to patients.

■ To bill for medical

  • Administrating and outsourcing of medical, nursing care, workers' compensation insurance and publicly-funded medical care at the Hospital.
  • Submitting receipts to reviewing and paying institutions.
  • Responding to inquiries from reviewing and paying institutions or insurers.
  • Submitting receipts to administrative agencies, etc., concerning publicly funded medical care, and responses to inquiries.
  • Other using for billing medical expenses related to medical care, nursing care, workers' compensation insurance, and publicly funded medical care.

■ To manage and operate the Hospital

  • Accounting.
  • Reporting of medical accidents, etc.
  • Improving medical services for patients.
  • Managing hospital admissions and discharges, etc.
  • Other using related to the management and operation of the Hospital.

■ To register patient cancer information

  • Submitting hospital cancer registries information to the National and Regional Cancer Registries.
  • Using hospital cancer registries for prognostic studies.
  • Conducting survival verification surveys by other medical institutions, resident registration inquiries, etc., when prognostic information cannot be obtained in the Hospital.
  • Submitting hospital cancer registries to the national compilation.
  • Participating in research using hospital cancer registries and DPC.
  • Providing information to organ-specific cancer registries based on hospital cancer registry information.

■ To Notify the results of health examinations, etc. commissioned by a company, etc. to the company, etc

■ To consult with or notify to professional medical associations, insurance companies, etc., concerning medical liability insurance, etc

■ To prepare basic data for the maintenance and improvement of medical and nursing care services and operations

■ To Cooperate for medical practical exercise conducted in the Hospital

■ To organize case conferences in the Hospital to improve the quality of medical care

■ To provide information to external audit organizations

■ To investigate of a patient's current and past medical records for the purpose of conducting clinical research or clinical trials or for the purpose of determining whether to conduct clinical research or clinical trials

Response to personal information protection

Please note that, as a general rule, patient calls, etc., will be handled as follows.

Calls for medical examinations, etc

For patient calls such as reception, examination, and accounting, only the number will be called by microphone.
Your name is called by voice.
At the time of account, your name is called by voice.

In-hospital broadcast calls

If we are unable to find you in case of emergency, we will call your name on the hospital broadcast.

People who receive an explanation of their symptoms

Basically, we will explain symptoms only to the patient himself/herself, but we may explain to family members and others when medically necessary.

Name display at the entrance of patient rooms

In order to provide safe and prompt medical care, we will display the patient's name at the entrance of the patient room.

Confirmation of admission or inquiring about inpatient rooms by phone

As a general rule, we cannot answer inquiries by phone.

Inquiries about inpatient rooms at the time of visitation requests, etc

As a general rule, general reception desk is open from 8:00 to 17:15 on weekdays, and the guard room is open on Saturdays, Sundays, national holidays (including substitute holidays), and during the year-end and New Year holidays to answer questions about the inpatient wards.
Please note that we may not be able to provide information on the patient's request.
In addition, there may be cases when a patient cannot be visited in the hospital ward due to his/her medical condition or other reasons.
